Whether one needs a brief rehabilitative stay or perhaps a longer stay for continuous care, the process of transitioning from a prior residence to a nursing home can be a difficult time for both the resident and family. Many conflicting emotions, including anxiety, fear, depression and even anger may be experienced.
The staff of CHCS will extend every necessary support, courtesy and assistance throughout the entire admissions process to ease the complicated feelings that are often involved and make certain the transition is as comfortable, reasonable and non-threatening as possible. Our caring Social Services team, comprised of experienced professionals, can assist elders and families with discharge plans to home as well as any adjustment issues. Our staff understands how challenging this time can be; they use their knowledge and expertise to help the elder person feel welcome and part of our community.
